An institution at the forefront of primary health in Puerto Rico. Consejo de Salud de la Comunidad de la Playa de Ponce, Inc. (CSCPP) was created by a group of community leaders as a non-profit community health center for the Playa Sector of Ponce, Puerto Rico, in 1971. It has been a HRSA grantee since inception and a Migrant Health Program (MHP) grantee since 1995. The CSCPP, for over 38 years, has delivered comprehensive primary healthcare services in four (4) communities of three (3) municipalities in Southern Puerto Rico, including the Playa (where the main center is located) and Tuque suburban sectors in Ponce, Tallaboa, a rural sector in the municipalities of Penuelas, and Singapur, a rural sector in the municipality of Juana Diaz, with a total of 263,725 inhabitants. The CSCPP's mission is to reduce health disparities and increase access to high quality comprehensive primary healthcare services for medically indigent and insured, underserved, vulnerable populations in impoverished targeted and underserved areas. The CSCPP has approximately 259 staff members for the main center and three outpost sites, as well as two schools under the School-Based Program. Its primary healthcare delivery model includes the following services: Pediatric, Internal Medicine, General Medicine, Family Medicine, Mental Health, Gynecology and Obstetrics, Minor Surgery, Urgency and Emergency, Dentistry and HIV services, as well as other collateral services that include: Pharmacy, Laboratory, X-Ray, Sonogram, Mammogram, Halter, Echocardiogram, Electrocardiogram, Vaccination, School clinics, Health certifications, Social work, Psychiatric, and Community Outreach. Health care practice is supported by collaborative partnership with higher educational institutions, regional heath care consortiums, community based organizations, state government agencies, and other healthcare networks.
